ABSTRACT
West syndrome is a developmental brain disorder. Children affected by this syndrome typically have delayed development compared to other children of the same age, but they do not have autism or intellectual disability. West syndrome may also be associated with symptoms of cerebral palsy as well as intellectual disability. Let’s know more about this rare uncommon condition that deteriorates the quality of life of the one suffering from it. The following article will provide information regarding the condition of west syndrome along with the description of its presenting features, causes, management and ayurvedic aspect.
INTRODUCTION
West syndrome is named after a renowned English physician William James West. He explained the case of his own son James Edwin West in an article published in 1841. West syndrome is also known as epileptic spasms, infantile epileptic encephalopathy, jackknife convulsions, salaam spasms, etc. Not much is known about what causes the syndrome, but genetic changes are thought to play a role in at least some cases. A specific genetic change called an "homozygous deletion" has been found in families with one child diagnosed with west syndrome and another without it. The description regarding the causes of west syndrome is mentioned as follows.
CAUSES
Causes of this syndrome are not known, but are thought to be related to certain genetic changes such as an "homozygous deletion" or "de novo point mutation". As it is a rare disorder, there have been few studies performed on those who have been diagnosed with it. However, it seems to occur mostly in males due to the fact that their primary symptoms are motor problems and delays in thought. A study conducted by Ardais et al. (2005) found that the percentage of males with a "de novo" mutation of a gene is 28% compared to 10% for females. This shows a higher occurrence of mutations in males than females. The study also found that the percentage of males with a deletion of this gene is 67%, but there were no females in the study with a deletion. This means that the chance of occurrence of the syndrome is much higher if males have this mutation. The presenting symptoms of the condition of west syndrome will be discussed as follows.
SYMPTOMS
West syndrome affects the development of the brain and central nervous system. Symptoms of west syndrome may include a triad of symptoms-
- Lightning Attacks - In this type of attack, sudden convulsions in the entire body occur within seconds or very rapidly and are severe in nature. Bent legs can be observed.
- Nodding Attacks - In this type of attack, throat and neck flexor muscles are the main part affected by convulsions. During this episode, the chin irregularly shifts towards the breast or the movement of head in the inward direction.
- Salaam or Jackknife Attacks - A rapid irregular bending of head and trunk of body can be observed along with the lifting and bending of the arms. He/she may also partially draw hands in front of the chest while raising the arms. This movement when observed or imagined in slow motion, may be correlated to a muslim greeting also called salaam, hence it is named salaam attacks also.
DIAGNOSIS & TREATMENT
Initial diagnosis and treatment involve identifying symptoms and working with special education teachers to help children learn communication, socialization, and self-help skills. The diagnosis may require different imaging tests such as CT scan (computed tomography), EEG (electroencephalography), MRI (Magnetic resonance imaging), etc. A "worksheet" can be used to help guide people who are working on developing goals for children with west syndrome. The allopathic medicinal treatment will include hormonal therapies such as adreno cortico tropic hormone (ACTH) and administration of steroids such as prednisone, etc. However, prolonged use of these drugs may result in various complications and hence it is not preferable. Epilepsy surgery may also be required to manage the condition if arising from a particular region of the brain. The ayurvedic aspect of this condition will be discussed as follows.
AYURVEDIC ASPECT
The ayurvedic principles help to provide a new and very unique perspective to the health conditions. West syndrome can be correlated to a condition similar to apasmara in the ayurveda. The vitiation of rajas and tamas are considered to be responsible for the neuro psychotic diseases. The apasmara as explained by the ayurvedic classical texts is presented with the following features and Bibhatsa chesta (tonic clonic involuntary movements) and tama pravesha (loss of consciousness) due to pragya apradha (Intellectual blasphemy) i.e. vibhransh (destruction or hindrance) of smriti, dhee, and dhriti (elements for intelligence). This imbalance in the mansik dosha will lead to imbalance between sharirik dosha (i.e. vata, pitta, kapha) also.
