INTRODUCTION

Arrhythmogenic right ventricular cardiomyopathy, as mentioned before, is a rare type of inherited condition which results in death at a young age. This condition appears in early adulthood. Arrhythmogenic right ventricular cardiomyopathy is a condition of myocardium. Myocardium is basically the muscular wall of the heart. The condition is responsible for breaking down the myocardium over time. This in turn increases the risk of an abnormal heartbeat called arrhythmia and can result in death as well. It may not show any type of symptoms but the individual is at the risk of death. A death which is sudden and quick. The risk increases when the affected individual is doing strenuous exercises.  When the symptoms of the condition occur it mostly includes fluttering and pounding in the chest which is understood as palpitations, lightheadedness along with syncope or fainting. Over time this condition can cause  breathlessness or shortness of breath along with swelling of abdomen and legs. If there is excessive damage to the myocardium then sudden heart failure will be noticed. CAUSES OF ARVC

The Causes of Arrhythmogenic right ventricular cardiomyopathy according to modern science is as follows:

Mutation

The mutation which can occur in at least 13 genes. Many of these genes are called desmosomal genes. They are named so because they provide information for the making of cell components which are called desmosomes. Desmosomes are known to attach heart muscles to each other thus are responsible to provide strength to the myocardium. Now we understand mutation in the desmosomes making genes directly impact the quality of myocardium. Much of the quality of myocardium is affected when the person is doing strenuous exercises or vigorous exercises. These changes generally affect the myocardium surrounding the ventricle of the right side. Ventricles are the two lower chambers of the heart where damaged myocardium is gradually replaced by fat and scar tissue.  Due to this abnormal tissue build up, the walls of the right ventricle stretches out and prevents the heart from pumping the blood. Thus the changes result in the electrical signal disruption which are responsible for controlling the heartbeat. This finally leads to arrhythmia.

Very less commonly there is a chance for mutation of a non desmosomal gene thus causing    Arrhythmogenic right ventricular cardiomyopathy.

The causes of the dosha aggravation are as follows:

  • Ati ushan - Consumption of hot food articles
  • Ati guru - Eating heavy food
  • Ati amla - Consuming of sour foods and taste
  • Ati kashaya - Astringent food articles
  • Ati tikta - Bitter food articles
  • Ati shrama - Increased physical exhaustion
  • Abhighata - Mental or physical trauma
  • Adhyashana - Excessive consumptions of food articles
  • Ati prasanga - Increased sexual activities
  • Sanchintana - Stress, thinking in excess along with over anxiety
  • Vega vidharana sandhaarana - Forcibly holding the natural urges
  • Vyamaya - Increased physical exercise
  • Tikshana or ati virechana - Administration of very strong purgation therapy

INHERITANCE OF ARVC

Up to half of all cases of Arrhythmogenic right ventricular cardiomyopathy tends to run in families. The most familiar cases of this disease have an autosomal dominant pattern. Autosomal dominant pattern means one copy of altered gene in each cell is sufficient to cause the disorder. Rarely, ARVC follows an autosomal recessive pattern of inheritance. The recessive pattern means when both copies of the altered gene in each cell have mutations.

SYMPTOMS OF ARVC

Symptoms of Arrhythmogenic right ventricular cardiomyopathy are as follows:

  • Arrhythmias - This is the abnormality in the timing pattern of a heart beat. This involves racing heart beat, palpitations or fluttering sensation.
  • Premature ventricular contractions -  Irregular and extra heart beats occur due to the disturbance in the electric signals in the lower chamber of the heart that is ventricle.
  • Ventricular tachycardia - This is a series of rapid heart beats which are originating in the ventricle. This may last for only a few beats or can last for longer creating a life threatening condition. The ventricular tachycardia is potent to cause weakness, nausea, vomiting and lightheadedness. This May also cause feelings of racing or skipping heart beat.
  • Syncope - This as maximum of the readers already know refers to sudden loss of consciousness and fainting.
  • Heart failure - In rare cases the patient shows symptoms of right heart failure and includes weakness, swelling over ankle and foot that is peripheral edema, built up of fluid in the abdominal cavity. The patient also suffers from arrhythmic symptoms as well.
  • Sudden cardiac arrest - This is a condition where the heart stops working. Here the beating and pumping both of the functions have stopped and can cause death if not treated in minutes.

On the other hand, Ayurveda explains the symptoms according to the dosha type. Like for vata dominant heart disorders pain is of stretching and pricking type. If severe then it can be of crushing type as well. The crushing type of pain is called nimarthana. The safotan or tearing pain is a characteristic of vata dominant heart conditions.

The heart conditions with pitta dosha dominance have features like polydipsia that is excessive thirst, increased body heat or daha that is burning along with chosha which is needling sensation. Haridya kalam is also a very important feature. Haridya kalam means fatigue of cardiac muscles along with mouth dryness.

Likewise in case of kapha dosha dominant heart conditions irregular heart beats are the commonest along with heaviness and anorexia. Fever, cough and drowsiness along with mukkha madhurya (sweet taste in mouth) are the commonest.

In case of Arrhythmogenic right ventricular cardiomyopathy there is involvement of all three dosha and hence we get a mixture of symptoms of vata, pitta and kapha dominance heart conditions or heart disorders.

DIAGNOSING ARVC

The definitive diagnosis of Arrhythmogenic right ventricular cardiomyopathy are done through:

  • A combination of non-invasive ECG and imaging evaluations
  • Cardiac MRI can also be done to rule out the anatomy of heart and motion of walls
  • Angiography with catheterization to visualise heart vessels and chambers
  • Electrophysiology studies are done to further analyse the heart rhythm

TREATMENT OF ARVC

The treatment of Arrhythmogenic right ventricular cardiomyopathy includes the following:

  • Medicine to control your heartbeat and rhythms like beta blockers
  • Antiarrhythmics are also given to control abnormal heart rhythms
  • Diuretics are also given to reduce water accumulation in the body
  • The ACE inhibitors are given to decrease the workload of the heart
  • Some blood thinners or anticoagulants are prescribed to prevent blood clots

In Ayurveda, there are many herbal, mineral and herbo mineral preparations given to the patients in order to support normal and healthy functioning of the heart. But the chikitsa sidhanta of the heart conditions starts as follows:

  • First of all, all the causative factors are to be avoided. This is called nidaan parivarjan.
  • Then complete mental and physical rest is to be provided to the patient
  • The patient should follow a healthy diet and lifestyle
  • According to dosha dominance the formulations are guided to the patient
  • In case of  three dosha involvement (sannipataj haridya roga) the first and foremost step is to guide the patient for the fasting
  • Tridosha pacifying diet is usually guided. Like old red rice, meat of wild animals, yellow lentils, soup of horsegram, errand oil, saindhav lavan, draksha, shunthi, kanji, honey, shweta kutaj and many others are to be administered to the patient.

The formulations which can be used in this condition are generally prepared using pushkarmool, arjun, palash, munnaka, rasana, go ghrita and certain minerals like  parad, gandhak, tamra bhasma, swarana bhasma, loha bhasma, shilajit, rajat bhasma and many others. These all help in the pacification of dosha and help in the regeneration recovery of myocardium.

The tablet formulations which are generally used in heart conditions are haridra arnav rasa, hridayi chintamani ras, chintamani ras, visheshwar rasa, kalyan sundar rasa, prabhakar vatt and trinetra ras. These are to be taken along with warm water.

The powder formulations which are mostly prescribed are pushkarmool churna, haritakyadi churna and pippali churna. The kwath/ arishta formulations are pushkaradi kwath, partharishtam, shunti kwath and many others.
