Pilonidal sinus is a tunnel or small hole in the skin of the top of buttocks area where they tend to divide (at tailbone). This area can also be termed as cleft. Usually it does not cause any signs or symptoms unless the infection invades. This abnormal pocket in the skin of buttocks area can contain skin and hair in it and take a form of cyst. This cyst can later become infected containing pus or fluid and turn into an abscess. Patient can have severe pain the particular area, as the infection and inflammation progresses. INTRODUCTION
Pilonidal sinus is an abnormal pocket in the skin above cleft region of buttocks which usually occurs in men as compared to women. These sinuses have tendency to reoccur as well. People involved in jobs which include long sitting period like drivers are more prone to develop Pilonidal sinus. These sinuses occur due to puncture of skin by hair where it becomes imbedded. Infection of Pilonidal sinus results in formation of Pilondial cyst that has debris, hair, pus or fluid in it and become extremely painful.
SIGNS AND SYMPTOMS
In early stages there are no signs or symptoms develop due to Pilonidal sinus only beside a small dimple near the cleft of buttocks which goes unnoticed but as infection occur and it develops into abscess, the symptoms are seen. Some of the common signs and symptoms are:
- Red inflamed skin
- Severe pain
- Draining of foul smelling pus
- Fever usually low grade
- Pain while standing or sitting
- Fluid, pus or blood oozing from the sinus
- Hair coming out from the sinus
- Tracts of sinuses developing over time
CAUSES
Long sitting activities causes a prolonged friction over buttock area which forces the hair growing around to go back into the skin. When this hair goes back into the skin our body recognizes it as a foreign material and launches a defense action against it leading to inflammation of the area. When this inflammation goes further, there occurs a formation of cyst which is prone to infection and as infection prevails, symptoms worsen. However, the exact cause of Pilondial cyst is not known but certain factors that can lead to it are:
- Hormonal changes during and after puberty
- Hair growth
- Repeated friction
- Long sitting jobs
Certain risk factors also increases the tendency to develop Pilonidal sinus like obesity, sedentary lifestyle, male gender, young age, and excessive hair growth on body.
DIAGNOSIS
Diagnosis is based on the clinical history and physical examination of the patient. A hole which usually has hair protruding present near top of cleft of the buttocks and filled with pus and blood indicates towards the Pilonidal sinus and further history confirms it.
TREATMENT
Usually the sinus is drained by making a small incision after numbing the area with local anesthesia. Pus is drained out and antibiotics are given to treat the infection. However the reoccurrence is very common and which can further be treated by more extensive surgery further but which again do not ensures no reoccurrence.
Ayurveda can help here by managing and treating the sinus completely and preventing its further occurrence. 1. Kaishore Guggul
