Alternative treatment for Non-Ischemic priapism with Herbal Remedies

Non-Ischemic priapism is erection of penis without any sexual stimulus or sexual desire. Non- Ischemic priapism or high blood flow priapism is a medical condition that occurs due to unregulated arterial blood flow within penis. This condition is less common and less painful as compared to ischemic priapism. The main reason behind this condition is blunt trauma to perineal region or directly to penis or scrotum. This article covers types of priapism, ethology, diagnosis and management as per Ayurveda. Non-Ischemic priapism

INTRODUCTION

As we have mentioned above Non-Ischemic priapism is a male disorder. Priapism is prolonged erection of penis which is without any sexual stimulus which last longer than 4 hours. Priapism is an uncommon condition which is not painful and but it is considered as a medical emergency. Priapism is of two types described below:

Ischemic

This type of priapism occurs due to lack of blood flow that leads to poor blood supply to chambers of corpus cavernosa resulting in painful erection. Males who are suffering from sickle cell anaemia, malaria, leukaemia, myeloma and who are on antidepressant therapy are more prone to this condition.

Non-ischaemic

This type of priapism occurs when blood flow is usually more than normal. Although this condition is rare than ischemic type. Non ischemic type mainly occurs as a result of trauma or injury to the penis or to perineum (Area between scrotum and anus). Any damage to the artery supplying blood to penis leads to Non-Ischaemic priapism. Non-Ischemic priapism if left untreated leads to progressive fibrosis and erectile dysfunction.

SYMPTOMS OF NON-ISCHEMIC PRIAPISM

Non-Ischemic priapism is a rare medical condition. It is persistent erection of penis occurs due to increased of blood. The symptoms of Non-Ischemic priapism vary in pain and blood flow as compare to ischemic type of Priapism.

  1. Penile erection more than 4 hours
  2. Absence of rigidity / flaccid erection
  3. Usually painless
  4. Erection without any sexual stimulus
  5. Presence of penile trauma or external injury

AETIOLOGY OF NON-ISCHEMIC PRIAPISM

In order to understand aetiology of non-ischemic priapism, firstly we should understand normal physiology behind penile erection. In normal condition, normal physiological or sexual stimulus causes contraction of blood vessels which leads to increased blood flow to the penis. Increased blood flow causes erection of penis, after stimulus is over erection comes to normal docile condition. Any kind of sexual stimulus initiate sending signals from brain to spinal cord that leads to penile erection. The erectile tissue corpus cavernosa is sponge like tissue which is filled with blood and responsible for erection. The root cause of non-ischemic priapism is improper blood flow to the penis. There are following factors which are responsible for unnatural blood flow to the penis:

  1. Any kind of external injury to perineum
  2. Spinal cord injury

The main cause of Non- Ischemic priapism is injury due to which excess amount of blood is pumped to penis resulting into its erection.

DIAGNOSIS

The diagnosis of any disease depends on various factors such as complete history, current and past medical history. Diagnosis of non-ischemic priapism is made on the basis of complete medical history and physical examination. The proper diagnosis is made on the basis of below investigations to know the type of Priapism:

  1. Color doppler examination
  2. Blood gas testing (High blood flow indicates increased level of Ph)

COMPLICATIONS OF NON-ISCHEMIC PRIAPISM

Non- Ischemic priapism is a rare condition but its progression can cause severe complications. When erection is present for longer time, it can cause destruction of erectile tissue. High blood flow can cause permanent damage of erectile tissue resulting in erectile dysfunction and progression of disease also leads to other sexual problems.
