Today again we came up with a rare blood disorder called Sideroblastic anemia in which due to structural deformity in the mitochondria's of Red blood cells, our body is unable to use iron and the person will end up with Anemia. Sideroblastic Anemia

INTRODUCTION

Sideroblastic anemia is a blood disorder where the iron is sufficient enough in the body but due to some defects the iron starts accumulating in the mitochondria's of red blood cells and gives rise to impaired structure of nucleus in mitochondria that is in the shape of ring hence called ringed sideroblasts. This structural deformity makes the mitochondria unable to utilize the iron for the production of hemoglobin hence interferes with the activities of the various organs and develops generalized weakness, tiredness, etc. This disease is also known by the name Sideroachrestic anemia.

This disease can affect anyone but the incidence ratio is quite high in males and more in the people below 25 and above 65 years of age.

AYURVEDIC ASPECT OF SIDEROBLASTIC ANEMIA

As per Ayurveda, it is a Tridoshaj vyadhi in which all the tridoshas (Vata, Pitta and Kapha) are involved. This disease is categorized under Pandu roga. In this disease Saptadhatus (All seven dhatus) especially rasa, rakta and majja dhatu, Trimalas (Three byproducts) along with Agni (Digestive fire) are involved and impairs the normal structure of the cells hence their functioning. Here, our Anuvanshika karna's (Hereditary factors), Sahaj Vikaras (Congenital defects) and Vyadhi kshamatva (Immunity) are involved.

ROLE OF HEMOGLOBIN AND MITOCHONDRIAS IN OUR BODY

Hemoglobin is an essential element in our body that plays a vital role in our body. It carries oxygen to the cells and brings back the carbon dioxide from the cells. So, that all the cells can perform their normal functions.

Mitochondria are an organelle which is considered as a powerhouse of the cell and provides energy to perform their normal functions in our body and it is present in the red blood cells. These mitochondria utilize the iron from the extracellular fluid for synthesizing cofactors for various functions in our body. They also convert Oxygen and nutrients into ATP (Adenosine triphosphate) which are responsible to carry out all the metabolic activities of the cells. Hence both the things synergistically affect the functions of cells and develop anemia.

WHAT CAUSES SIDEROBLASTIC ANEMIA?

The causes of this disease are still idiopathic. But there are some factors which are responsible for this such as:

  • Congenital defect - Any kind of defects in bone marrow, chromosomal abnormalities, etc.
  • Hereditary factors - X linked recessive genes, chromosomal abnormalities, etc.
  • Acquired causes are - indulgence in the long term use of some steroids, immunosuppressants and any malignancies.
  • Mutation in the genes is also a causative factor for this disease
  • Long term history of Diabetes, HTN, etc will act as a triggering factor for this disease.

SIGNS AND SYMPTOMS OF SIDEROBLASTIC ANEMIA

These are some common signs and symptoms of this disease:-

  • Fatigue
  • Tiredness without work
  • Generalized weakness
  • Gasping for air or breathlessness
  • Paleness of skin, nails and eyes.
  • Blurred vision
  • Tachycardia (Increased heart rate) and palpitations
  • Dizziness
  • Vertigo
  • Inability to carry out daily works
  • Reduced cognitive skills
  • Delayed milestones
  • Sluggish activities
  • Chest pain
  • Headache

DIAGNOSIS

It is required to go for thorough investigation related to blood.

  • Complete blood count (CBC)
  • Serum iron evaluations
  • Ferritin levels
