What is Schizoaffective Disorder (SAD)?

Schizoaffective disorder is a chronic psychiatric disorder which is an amalgamation of two separate disorders i.e. Schizophrenia and Mood disorder. Schizophrenia is characterized by delusions and hallucinations which strikes between 16 and 30 of age affecting 1% of the population. Whereas mood disorder is symbolized by unexpected change in mood periodically which affects 1-2% of general population. According to National Institute of Mental Health and Diagnostic & Statistical Manual (DSM) of mental disorders (5th edition), schizoaffective disorder affects 1% of the total population.

Types of Schizoaffective Disorder:-

1. Bipolar Type

Bipolar disorder includes manic episodes which is characterized by racing thoughts, euphoria, excessively good mood, risky behavior (high sex drive and aggression) and impatience.

2. Depressive Type

In Depression affected person experiences diligent sad mood, anxiety, low energy level, suicidal thoughts and sleeping disorders.

What are the Signs and Symptoms?

Signs and symptoms of schizoaffective disorder may vary from individual to individual. Some of the major symptoms are as follows:-

  • Delusions 
  • Lack of motivation
  • Paranoid thoughts
  • Disorganized thoughts or thinking
  • Catatonic behavior (inability to walk normally) or grossly disorganized.
  • Suicidal thoughts
  • Sleeping disorders
  • Sudden change in appetite
  • Problem in managing personal care i.e. poor hygiene, cleanliness, physical appearance.
  • Hyperactive behavior
  • Antisocial behavior
  • Lack of concentration

Causes of Schizoaffective Disorder

Schizoaffective disorder is considered as the most misdiagnosed psychiatric disorder in the history of clinical practices. Some of the causes of this disorder are discussed as follows:-

  • Genetic inheritance – Schizoaffective disorder has the tendency to run in families according to the medical surveys there is more than 5% of probability of child getting affected if one of the parent is diagnosed with schizoaffective disorder.
  • Chemical imbalance in brain – Imbalance of dopamine and serotonin which are the neurotransmitters triggers the onset of this disorder.
  • Environmental factors and stress - Stressful events in life which includes abuse, feeling of loneliness, financial problems, death of someone close or traumatic loss can lead to severe psychiatric disorders.
  • Drug induced schizophrenia - Consuming drugs such as marijuana or LSD are considered to induce schizophrenia symptoms and triggers the dopamine and serotonin production which directly or indirectly causes the onset of schizoaffective disorder. 
  • Childhood trauma - It is believed that if a child experiences emotional distress he/she may develop bipolar disorder. These experiences includes sexual or physical abuse and traumatic events such as death or loosing someone or have faced rejections or neglecting behavior.

Complications in Schizoaffective disorder may include following:-

  • Drug addiction and abuse
  • Suicidal thoughts and behavior
  • Maniac behavior  
  • Difficulty in following medication schedule/regimen

Clinical Diagnosis of Schizoaffective Disorder

Schizoaffective disorder is difficult to diagnose clinically due to its interdisciplinary nature. Some of the diagnostic tests are given below:-

  • Complete blood count (CBC)
  • Thyroid-stimulating hormone level (TSH) level
  • Rapid plasma reagent
  • Urinalysis
  • Urine drug screening test
  • HIV test
  • Lipid panel
  • Brain MRI, EEG or CT scanning
