A Colovesicular Fistula is an uncommon condition characterised by an open connection between the large intestine (colon) and the bladder which allows the fecal matter from the colon to enter the bladder causing infections which are very painful as well as other complications.
The function of the colon is to release stool through the rectum. In this condition, the colon sits above the bladder. The function of the bladder is to store urine before it is released through the urethra. There is a separation of colon and bladder through a thick wall of tissue.
Trauma or surgery to this part of the body can cause the fistula to form. When an opening develops, it results into colovesical fistula.
It occurs when a phlegmon or abscess extends or ruptures into the adjacent bladder. It can sometimes be managed conservatively because patients do not want an operation or sometimes they are at high surgical risk.
This condition is treatable. However, it is so unusual so there is a limited amount of information available to manage this painful condition.
WHAT ARE THE SYMPTOMS OF COLOVESICULAR FISTULA?
- The most common symptom of colovesicular fistula is pneumaturia which occurs when the gas from the colon mixes with the urine. Patient experiences bubbles in the urine.
- Painful or burning sensation when the person urinates and recurrent Urinary Tract Infections which develop due to an irritation of the bladder but nearly half of the patients of colovesical fistula complaint about this problem.
- When fecal matter mixes with urine, there is an appearance of brownish or cloudiness in the urine.
- Blood in the urine.
WHAT ARE THE CAUSES OF COLOVESICULAR FISTULA?
More than half of the cases of colovesicular fistula are the result of diverticular disease. The most common cause of colovesicular fistula is diverticular disease followed by malignancy and Crohn's disease.
Other Causes Include:
- Inflammatory Bowel Disease particularly Crohn’s disease.
- Colorectal Cancer
- Surgery that involves the colon or bladder
- Cancer of the other surrounding organs
DIAGNOSIS
Colovesicular Fistula is diagnosed with an imaging test known as Cystography. In this procedure, a thin and flexible tube with a camera is inserted at one end into the bladder. The camera relays the image of the bladder wall to a computer so that the doctor can diagnose fistula.
TREATMENT
Treatment generally includes being fed through the veins so that the intestines do not have to work, prescription of antibiotics as well as steroids, catheter being inserted into the bladder to drain fluid that may have seeped into it from the colon.
