AN OVERVIEW

Generalised epilepsy with febrile seizures plus (GEFS+) is an unusual, inherited epilepsy syndrome which comprises a range of seizure disorders with varying severity. In other words, this condition can be described as a syndromic autosomal dominant disorder in which several epilepsy phenotypes can be exhibited by the afflicted individuals.

Generalised epilepsy with febrile seizures plus (GEFS+) characteristically causes febrile seizures and spontaneous seizures which can mostly be managed with the help of traditional antiepileptic drug therapy. The condition is typically associated with families in which several members belonging to different generations suffer from different types of epileptic seizures and probably even various epilepsy syndromes.

In addition, some of the other epilepsy seizures which may extend beyond 6 years of age include myoclonic seizures (which cause involuntary twitching of muscles); absence seizures (which cause ‘staring spells’ or a brief loss of consciousness); and atonic seizures (which involve ‘drop attacks’ or occasional incidents of weakened muscle tone).

The description of GEFS+ as an epilepsy syndrome in its own right is a fairly recent development. As a result of the categorization of GEFS+ as an epilepsy syndrome, there are now three epilepsy disorders which are grouped under this condition:

  • SMEI (Severe myoclonic epilepsy of infancy or Dravet's syndrome)
  • SMEB (borderline SMEI)
  • IEC (Intractable epilepsy of childhood).

CAUSATIVE GENES

The commonly-perceived cause of Generalised epilepsy with febrile seizures plus (GEFS+) is: mutations (faults) in the sodium channel α subunit gene SCN1A.

Besides the SCN1A gene, there are some other genes which are also considered as the causative genes for the extremely complex GEFS+ disorder. These genes include: sodium channel β subunit gene SCN1B; a GABAA receptor γ subunit gene; the GABRG2 gene; and the PCDH19 gene related with calcium channel.

Overall, there are more than 20 different gene mutations which have been identified in the patients of GEFS+ and their family members.

DIAGNOSIS

WHAT IS PLEOMORPHIC ADENOMA?

Pleomorphic Adenoma is a rare type of tumor in the salivary gland. This tumor is very common and can develop in any salivary gland- mouth, neck or throat. Most of the times, tumors that form on the glands are noncancerous but the tumors on the salivary gland are malignant. It is in the form of an asymptomatic painless mass that grows gradually.

TYPES

We have many tiny salivary glands on the lips, inside cheeks, and mouth and throat. There are three main salivary glands under and behind the jaw — Parotid, Sublingual and Submandibular.

Parotid gland is the salivary gland in which most tumors occur. Nearly 25 percent of parotid tumors are cancerous.

Ayurvedic Treatment for Azotemia - Types, Symptoms, Diagnosis, Herbal Remedies & Diet

Azotemia is a condition when the kidneys are unable to get rid of enough nitrogen wastes and they get build up in the blood. The kidneys are damaged due to some disease or an injury and so they are not able to filter the blood properly.

Azotemia is defined as an increase in urea nitrogen and creatinine. It is a type of nephrotoxicity.

IT IS OF THREE TYPES

1. Prerenal Azotemia

Prerenal azotemia is the most common type of azotemia and can be reversed. This condition occurs when something that is affecting the blood circulation affects the kidney function and the fluid doesn’t flow enough through the kidneys. For example when there is low blood pressure to the kidney or due to irregular heart function, the kidney is unable to filter out the waste products or make urine. This leads to high levels of concentration of serum creatinine and urea. Other reasons that may cause reduced blood flow to the kidney are burns, dehydration, hemorrhage, long term vomiting or diarrhea, too much exposure to heat, shock or any blockage of the artery that supplies blood to the kidneys.

2. Intrarenal Azotemia

Intrarenal azotemia is also known as acute renal failure or acute kidney injury. It is attributed to the problem with the kidney itself like infections, sepsis or disease of kidney. The most common type of intrinsic azotemia or intrinsic renal failure is acute tubular necrosis. In this case, there is excess nitrogen in the blood due to side effects of certain medications like antibiotics, anti-fungal drugs, chemotherapy, bigological therapies etc. It can be due to diabetes also.

3. Postrenal Azotemia

Postrenal azotemia happens due to obstruction of urinary tract leading to renal failure. It can occur along with prerenal azotemia. It happens when there is a blockage in the urinary system like stones, tumors, infections, urinary catheter, urinary crystals or enlarged prostate gland.

Azotemia is common in people aged between 45 to 65 years.

Long term diarrhea, vomiting, bleeding can also lead to azotemia.

SYMPTOMS OF AZOTEMIA

  • Painful urination which can be infrequent and decreased in quantity
  • Discoloration or urine and pale skin
  • Muscle weakness and fatigue or loss of energy and fast pulse rate
  • Fever, chills, nausea and vomiting
  • Loss of appetite along with feeling of dry mouth

  • Peripheral edema or fluid retention

DIAGNOSIS OF AZOTEMIA

Azotemia is diagnosed by urine and blood tests. The tests are done to check the urea nitrogen and creatinine levels.

The doctor takes complete medical history of the patient and record the medications, past infections and symptoms. He does physical exam too. At times, ultrasound and x rays may also be performed. A catheter may also be inserted into the urethra to investigate.

DIET RECOMMENDED FOR AZOTEMIA

The patients are advised to increase vegetables and fiber in diet. Vegetables should be cooked properly with lot of water.

They need to avoid excess of potassium and magnesium and even monitor the protein intake. Foods rich in potassium are bananas, potatoes, tomatoes, beans, chocolate, nuts, peaches, oranges, milk, raisin etc. They need to be limited in diet. Too much protein also makes kidneys work harder so the intake of protein should be balanced.

Sodium should also be taken in limits as it caused high blood pressure and swelling.

Carbohydrates intake should also be decreased to control the blood glucose levels. It is recommended to avoid junk foods, cheese, fruit juices and caffeine. Fluid intake has to be restricted as amount of urine is decreased in this problem.

High cholesterol diet should be avoided as they may cause kidney damage and heart problems.

Radish juice is very beneficial in this problem.

Taking a spoon of basil / tulsi juice with honey everyday is suggested.

A paraphilia is an emotional disorder in which a person's sexual arousal and gratification depends on fantasizing about sexual activities and engaging in sexual behavior that is atypical and extreme. The patient has fantasies, urges, behaviors that is persistent, intense, and occur over a period of at least six months. This problem causes distress and interferes in patient's day to day life. The behavior involves inanimate objects, children or nonconsenting adults and leads to suffering or humiliation for the patient. There could be sexual arousals to atypical objects, situations and targets like corpses and animals too. They have potential to harm others, too.

The word paraphilic is derived from para which means around or beside and philia means love.

CAUSES OF DEVELOPING PARAPHILIC DISORDER

Paraphilias is a disorder mostly diagnosed in men during adolescence and it reduces as the individual ages. Most people who develop a paraphilia begin having fantasies about it before they are 13 years old.

There could be biological, psychological and social risk factors for developing paraphilias. There is some evidence of altered brain functioning and functional anatomy in some paraphiliacs. They could have gone through an early emotional trauma. Other causes resulting in Paraphilic disorder are brain injury, schizophrenia, biological factors, hormones. Some have a childhood that can lead the individual to associate sexual pleasure with that object or events resulting in developing paraphilia.

SYMPTOMS OF PARAPHILIC DISORDER

A person with paraphilia feels personal distress about their interests and have a sexual desire or behavior that involves another person’s psychological distress, injury or death. They have an impaired or a nonexistent capacity for affectionate, reciprocal emotional and sexual intimacy with a consenting partner.

Pain, beating or spanking, depression, anxiety.

Symptoms of paraphilia may include preoccupation to the point of obsessiveness.

The patient feels a release of tension afterwards but also feels guilt and remorse.

Paraphilic disorder includes eight conditions : exhibitionistic disorder(exposing the genitals), fetishistic disorder(using nonliving objects ), frotteuristic disorder (touching or rubbing against a nonconsenting individual), pedophilic disorder(sexual focus on children),  sexual masochism disorder( undergoing humiliation, bondage or suffering), sexual sadism disorder (inflicting humiliation, bondage or suffering) , transvestic disorder (engaging in sexually arousing cross dressing )  and voyeuristic disorder (spying on others engaged in private activities).

DIAGNOSIS OF PARAPHILIC DISORDER

Psychiatrists, psychologists and mental health therapists are the ones consulted in diagnosing the paraphilic disorder.

Anti-Müllerian Hormone (AMH) levels and its effect in fertility of females

ANTI-MÜLLERIAN HORMONE (AMH) LEVELS: OVERVIEW

'AMH' is an acronym for 'Anti-Mullerian Hormone' which is produced directly by the ovarian follicles. Specifically, AMH -- also known as MIS (Mullerian Inhibiting Substance) -- is produced by the granulosa cells which comprise the lining of the ovarian follicles. The secretion of AMH is largely related to small antral follicles in the ovary which are sized between 5mm and 8mm.

Trichonodosis (also called Trichorrhexis nodosa) is a common hair shaft defect. The condition is typically characterized by knotted hair on the distal portion of the hair shaft, and results in the thickenings or weak points (nodes) due to the hair break off easily. This condition can lead to hair loss, damaged-looking hair, and lack of hair growth.

Trichonodosis is basically a ‘focal defect in the hair fiber.’ It results from the absence of the cuticle which encloses the hair cortex in a strong layer.

The absence of the cuticle directly exposes the hair cortex to the physical and chemical factors in the environment, and triggers the breaking down of the chemical bonds which are responsible for maintaining the hair structure; thereby making the hair weak. The weakening of the hair means that it can split and fray into minute strands at the point of the breakdown of the cuticle.

Morphea is a disorder characterized by an area of inflammation and fibrosis or thickening and hardening of skin or subcutaneous tissues due to increase collagen deposition. It is a rare skin disease causing painless, discolored oval shaped patches on the skin. It is an autoimmune disease and might be triggered by radiation therapy or repeated trauma to the affected area, surgery, extreme exercise, or any recent infection or any environment exposure.

Morphea is not contagious.

Morphea lesions can be subdivided into following subtypes according to the clinical presentation and depth of tissue involvement.

Circumscribed or limited: few circle are seen on the trunk or limbs. It is limited to one or several patches. It is also called Plaque Morphea. This is the most common form with 3-4 oval lesions. The lesions are painless but might itch. The patches are 1 to 20 cm in diameter.

Generalized: Many circles are seen on the trunk or limbs. It is spread over larger areas of the body. It is more severe form of localized disease and might lead to disfigurement. The lesions may join together.

Linear: Lines of involvement on the limbs or head. It can be seen in early age of 20 years. It can affect not just the skin but underlying structures such as muscle and bone. It features a single band of thickened, discolored skin. If linear morphea occurs on the face, it might cause issues with eyes or alignment of teeth in school ages children.

Mixed: Combination of circumscribed and linear or generalized and linear.

Pansclerotic: Involvement of all of the skin. It is rapidly progressive type of morphea with many plaques. It spares hands and feet.

In this disorder, the skin changes appear on the abdomen, chest or back. It might happen on the face, arms or legs in some cases. It may also involve the inside of the mouth, genitals and the eyes. But it is mainly on the outermost layer of the skin. In rare cases, it may restrict the movement of the joints.

Morphea often occurs first in childhood or middle adulthood. It affects women more than men.

Symptoms of Morphea

Symptoms of Morphea depends on the type and stage/severity of the disorder. It is mainly asymptomatic, with occasional itching and pain.

The most visible symptom of Morphea is a reddish, purplish oval patch of skin. These patches gradually develop a lighter or whitish center. The skin then becomes hard, thickened, dry and shiny.

Loss of hair and sweat glands in the affected area is visible.

In children, the linear morphea on arms or legs can affect the underlying bone and interfere with bone growth. If on the head, it may extend inwards to the brain and causes headaches, nerve palsies or seizures.

If morphea crosses the joints, the thickening may limit joint movement.

Constriction of the chest wall leads to shortness of breath in case of pansclerotic morphea.

Oral and dental problems occur in case of teeth and jaw involvement in linear morphea. It leads to difficulty in chewing, jaw locking and pain.

Other symptoms include fatigue, muscle pains. Heartburn, eye dryness, cold hands etc.

Diagnosis of Morphea

Diagnoses of Morphea is done by doing various blood tests, skin biopsy and MRI.

A complete medical history and family health history is taken by the doctor.
