Glandular cheilitis is a rare inflammatory disorder of the lip which is characterised by swelling of the lip with variable inflammation and the secretion of a clear and thick mucus. The chronic exposure of the lower lip becomes affected by the environment which leads to erosion, crusting, ulceration and infection. It is more common in adult males as compared to females and children.
Complaints of the patients suffering from glandularis cheilitis vary according to the nature and degree of pain, the enlargement and the loss of elasticity of the lip and the extent of evident surface change. Sometimes, a burning discomfort or sensation of rawness referable to the vermillion border may be reported which is associated with erosion, frank ulceration with crusting.
WHAT ARE THE SYMPTOMS OF GLANDULAR CHEILITIS?
- Swelling and pouting of the lower lip.
- Numerous pin sized orifices from which saliva can be easily expressed.
- Tenderness
- Uneven surface of the lip
- Salivary gland duct orifices may be dilated and appear as red or black in colour.
GLANDULARIS CHEILITIS IS SUB CLASSIFIED INTO 3 TYPES
- In the simple type, multiple and painless papular surface with central depressions and dilated canals are seen.
- The second or superficial suppurative type consists of painless, indurated swelling of the lip with shallow ulceration and crusting.
- The third type consists of a deep-seated infection with the formation of abscesses, sinus tracts or fistulas which are potential for scarring.
WHAT ARE THE CAUSES OF GLANDULAR CHEILITIS?
- Lip enlargement is due to inflammation, edema, hyperemia and fibrosis.
- It is an unusual clinical manifestation of cheilitis that evolves in response to one or more diverse sources of chronic irritation.
- Erosion, surface keratosis and crusting develop due to longstanding actinic exposure or due to excessive wetting due to compulsive licking, dryness or any other repeated stimulus that could serve as a chronic aggravating factor.
WHAT ARE THE COMPLICATIONS OF GLANDULAR CHEILITIS?
- Squamous cell carcinoma may arise particular in the context of long term sun exposure and smoking.
- Secondary bacterial infection can result in drainage of pus, ulceration and abscess formation.
TREATMENT
According to the modern medicine, the treatment of glandular cheilitis is not necessary because it is not successful in restoring the lip back to the normal but if it is caused by sun damage or infection, then treatment is necessary.
Treatment Include:
- Prescription of topical or oral antibiotics
- Excision of a nodule if there is a suspicion of squamous cell carcinoma.
- Vermilionectomy i.e. the excision of the entire affected lip.
