Kidneys and its Functions

Kidneys are bean shaped organs located retroperitoneally on either side of the vertebral column. These are the biggest drainage system of the body which flushes out all the toxins and waste products of metabolism that are produced inside the body.

What is chronic kidney disease?

Kidney disease is better known as chronic kidney disease. It is characterized by a gradual loss of function of both the kidneys over a period of time. Initially it may remain asymptomatic after which symptoms of a full blown kidney disease are evident. Chronic kidney disease if left unchecked or unnoticed may lead to permanent loss of kidney function leading to kidney failure which is an irreversible condition.

How diabetes can affect the kidneys?

This condition is also called as diabetic kidney disease or “Diabetic nephropathy”. In people with persistently high blood sugar levels (Diabetics), there is damage to the glomeruli which leads to micro seepage of albumin proteins from the blood into the urine. This is called as “micro albuminuria”. If left untreated it may progress to macro albuminuria or “Proteinuria” in which large amounts of proteins are passed into the urine. This leads to a slowly progressive development of kidney disease. Diabetic kidney disease in fact, is one of the major causes of kidney failure today. It affects both type -1 and type -2 diabetics.

How does high blood pressure affect kidneys?

The kidneys have a dense network of arteries, veins and capillaries around them. The kidneys of a hypertensive patient consistently receive blood at this raised pressure in its surrounding blood vessels. This continuously causes damage to these blood vessels which are not able to function as efficiently and get damaged, and may thicken and harden over time. This in turns affects the blood supply to the nephrons damaging them as well. As a result there is a slow and gradual impairment of the renal function leading to CKD and ESRD in later stages.

What are the other causes of kidney disease?

Besides Diabetes and Hypertension (High Blood Pressure), there are a number of factors and disease which lead to chronic kidney disease and then ESRD. These are listed below:-
  • Congenital malformations of the kidney
  • Poly cystic kidney disease
  • Glomerulonephritis
  • Pyelonephritis
  • Illegitimate use of drugs like Lithium, NSAIDs and Aspirin
  • Blocked channels in conditions like renal stones and BPH.
  • Viral infections like - HIV, AIDS, Hepatitis - B, C.

What are the symptoms of kidney disease?

What is the Diagnosis for kidney failure?

Various tests are recommended on the basis of clinical findings, signs and symptoms on the basis of which the final diagnosis of kidney failure and the degree of its damage and prognosis are decided. The tests are as follows:-

II. SERUM CREATININE

A serum creatinine level of > 1.2 for women and > 1.4 for men indicates kidney disease. The value rises with progressive renal impairment.

III. BLOOD UREA NITROGEN

The normal value is 7 - 20. As the disease progresses, the value may rise consistently.

URINE TESTS

I. URINEANALYSIS

This is a dipstick test which is used to analyze the constituents present in the urine sample such as blood cells, sugar, pus cells, proteins etc. This test is a routine examination carried out in disease like Diabetes, Urinary tract infections, CKD, bladder and kidney stones.

II. URINE PROTEIN

To determine the albumin: creatinine ratio in the urine sample.

III. MICROALBUMINURIA

To determine the amount of albumin in urine, which is common in diseases like CKD and Nephrotic syndrome.

IV. CREATININE CLEARANCE

This determines how much blood is being filtered through the kidneys.

IMGAING TESTS

I. ULTRASONOGRAPHY

It helps to determine the size, shape and presence of any calculi or tumor in the kidneys.

II. C.T. SCAN

It is an advanced technique used to determine the abnormal structural configuration of the kidneys and their functional inefficiency.

III. KIDNEY BIOPSY

It is done to ensure the extent of damage and whether a kidney transplant is essential or not.

What modern medicines have for kidney disease?

There is no medicinal cure for a depleting renal function in modern medicines. The line of treatment is based only upon the management of symptoms like High blood pressure and high cholesterol levels. In addition, a stage - 4, 5 CKD patient generally may be considered for a kidney transplant.
Once the condition of Kidney failure is established, the only options remaining are Dialysis, Kidney transplant and certain lifestyle modifications. The first two are highly invasive and expensive and lifestyle modifications are an adjuvant therapy that varies from person to person in terms of the degree of response.

What is dialysis?

In people having kidney failure, the kidneys no longer perform their function of removal of excess wastes and water from the body. In such patients the process of dialysis helps by artificially taking over the function of the kidneys. It is done in case of both acute and chronic renal failure. It is done by two ways and hence is of two types:-

1. HAEMODIALYSIS:

It is done by making an intra-venous line through a blood vessel in the arm. The blood from this line, known as the fistula is passed into the hemodialyzer and is filtered off the various toxins and impurities present in it.

2. PERITONEAL DIALYSIS:

In this condition the blood is cleansed within the body by attaching a catheter to the abdominal wall. A fluid called dialysate is filled in the peritoneal cavity and filters out the waste products from the surrounding blood vessels.

Kidney transplant

It is the process of replacing the damaged kidney of a patient of ESRD (End stage renal disease) with a normal working one taken from another individual. This is the best alternative for a patient with total kidney failure, the other one is dialysis, which though an effective procedure but a very tedious one and painful on the part of the patient. Kidney transplantation also has its own demerits as it is also an expensive procedure and requires kidney that has a matching allograft. Unmatched kidneys are rejected by the body tissues resulting in failure of the whole process. Other than this, it is only performed under the supervision of a specialist doctor who is capable enough to carry out the process effectively.

Ayurveda and kidney disease

Chronic kidney disease is a condition of the mutravaha srotas (The body channel that is related to the urinary system of the body and its normal functioning). The damage to the tissues is contributed by the imbalance in the three doshas (body humors - Air, Fire and Ether/ Vata, Pitta and Kapha) mainly the vitiated Vata element and the raised Kapha element which leads to the blockage of various channels of the Mutra vaha srotasa leading to impaired functioning of the kidneys.
In the above discussion, we have thrown light up on the modern methods of managing CKD and ESRD through various techniques. They have their own set of difficulties that they pose on the patient and the adverse effects associated with them. This is where Ayurvedic treatments come to the rescue. They have a miraculous blend of natural medicinal herbs which rejuvenate the kidney tissues and also boost the effective production of urine output.   • Drink plenty of water but only in ample amounts. Don't overdo it if you don't feel thirsty. Prefer taking Luke warm or just normal water.
  • Don't hold on to your urge to urinate. Keep on evacuating the bladder at regular intervals.
  • Keep the genitalia and pubic area clean to prevent catching any secondary infection.
  • Manage blood sugar levels if you are diabetic. High blood sugar levels are detrimental to the kidney function and may cause rapid deterioration of kidney function.
  • Avoid medications that may be unknowingly harming the kidney tissues, for example, NSAIDs.
  • Refrain from the use of alcohol and tobacco. They both cause kidney damage.
  • Exercise regularly according to the capacity.
  • Manage blood pressure well and try lead a stress free life, maintaining a positive attitude in every situation.

DIET

  • Drink fresh fruits and vegetable juices in plenty. Prefer herbal tea instead of the regular ones.
  • Avoid food products that are rich in potassium and sodium. These include table salt, potatoes, peas, nuts, beans, dry fruits etc.
  • Avoid use of too much fat in diet as CKD patients are prone to hypercholesterolemia.
  • Take the right amount of protein in diet. Green gram, lentil, pink lentil are some of the safe proteins that kidney patients should prefer. Avoid meat, eggs and non-veg food as far as possible. Red meat should be completely prohibited. Same goes for packaged and frozen foods and high sugar based artificial products.
  • Include a lot of vegetables, fruits and whole grains in diet such as, Pumpkin, Radish, Carrot, Onion, Eggplant, Cucumber, Lettuce, Turnip etc.
  • Fruits include Papaya, Watermelon, Apricots, Amla, Bael, Grapes, Blueberry, Olives, peaches, pears, figs, and Cranberry etc.
  • Avoid use of spices also if potassium levels are deranged.
  • Vitamin-D is also recommended in such patients.
  • Dont drink fruit juices as they are rich in Potassium.

Alternative Treatment for Ulcerative Colitis


ULCERATIVE COLITIS - AN OVERVIEW OF THE DISEASE

INFLAMMATORY BOWEL DISEASE

It is a type of inflammatory bowel disease i.e. it involves inflammation of a part or whole of the intestine. Crohn’s disease and ulcerative colitis are both grouped under this heading as they both affect the bowel in different ways leading to inflammation and ulcerations in the intestinal wall.

CROHN’S DISEASE AND ULCERATIVE COLITIS – WHAT IS THE DIFFERENCE?

As stated earlier, both these diseases are grouped under inflammatory bowel disease, the major difference is that while Crohn’s disease affects the intestine as a whole ulcerative colitis is the inflammation and ulceration of just the rectum and colon part of the intestine.
                           
ULCERATIVE COLITIS IS NOT IRRITABLE BOWEL SYNDROME (IBS)

IBS does not involve inflammation of the bowel and is also different from IBD in that it only affects the nature of peristalsis in the patient’s intestine. It should therefore, not be confused with ulcerative colitis.

IS ULCERATIVE COLITIS AN AUTO-IMMUNE DISORDER?

There are several assumptions and theories regarding the cause of this disease. The most accepted and popular one suggests an involvement of the immune system which goes haywire and starts attacking the healthy tissues of the rectum and colon instead of infections and other things that triggers the immune system to release antibodies.
It is unclear however, that what triggers the shift of this immune reaction. The possible understanding can only be that it is a result of some genetic response or probably an environmental effect.

ONSET OF THE DISEASE

The disease can affect any age group but most commonly it starts showing up between the age group of 15 – 26.
The ratio of women to men affected by the disease is 1:1. Its occurrence is however varied in different races groups. The Asians are said to be the most resistant to developing this disease. Caucasians and blacks are the ones who are most commonly affected by this disease.

SYMPTOMS

·       Bleeding from the rectum
·       Increased frequency of passage of stools
·       Mucoid discharge from the rectum
·       A continuous urge to evacuate the bowel
·        The disease has a gradual onset
·        Severe pain in the lower abdomen with cramps in serious cases

·       BARIUM ENEMA X-ray is also a confirmatory test for the diagnosis of ulcerative colitis.

·       CT scan and MRI also lead to significant findings required for the treatment of the disease.

The main idea behind the management of ulcerative colitis is the control of the symptoms and prevention of recurrence.

Ayurvedic treatment for Lichen sclerosis


This is a complicated disorder when the patient suffers a lot and keep applying various steroid creams and does not get relief. 

The inflamed area gets worse and worse and the skin becomes thinner and thinner leading to more itching, scratching and burning sensation. 

The sexual intercourse can also be very painful and sometimes can cause bleeding. 

According to Ayurveda, the treatment of Lichen sclerosis is focussed mainly on purifying the blood. 
According to aetiopathylogy as per Ayurveda-  It is  Vata in Rakta with Pitta imbalance ( especially Bhrajaka pitta) 

The reason is consumption of too much sour, salty, dry, stale food. Frozen, packaged food, packaged sour juices, late night sleep and consuming cold water or cold drinks along with the food.

The Vata and pitta gets vitiated and vata dries up the place which is weaker in the body. This is called "Kha-Vaigunaya concept" in Ayurveda. 

The weakest part is affected and the dosha accumulate there and affect the "Dushya" which is Rakta and Skin (Tvak) 

The treatment is pacifying Vata, purifying the Rakta ( Blood) and Pacifying the Pitta. 

Avoid - Sour fruits, Salty, pungent food, SUGAR, Alcohol, Non-Veg diet- Red meat, beef, pork,  Cheese, Milk and Milk products, Chillies, Spicy food, Beer, Wine, Smoking, Yogurt, Pineapples, Lemons, Kiwis, Biscuits, Salted snacks, Chips, wafers etc. Also avoid white flour and Sugar. Avoid Tomatoes, Pizza and Pasta and diet rich in Cheese. 

Advised -  Consume more vegetables like zuchini, Corn, Long gourd, bitter gourd, round gourds, pumpkin. Pumpkin is the best. 
Coconut water is excellent. Drink everyday. 
Turnips are good, Carrots, Radish, Beetroot, Cabbage, Peas, Potatoes are ok ( Don't take fried potatoes or french fries ) 

There are certain herbs and ayurvedic formulations for lichen sclerosis, which can be very helpful.

Mix all together, make 60 sachets, consume 1 sachet twice daily with 1 tablespoonful of Honey + 4 drops of almond oil ) 

2. Cap. Neem - 2 twice daily
3. Tab. Purim - 2 twice daily
4. Cap. Curcumin -2 twice daily
5. Tab. Kishore Guggul -2 twice daily
6. Tab. Purodil - 2 twice daily

Ayurvedic Treatment of Psoriasis

PSORIASIS TREATMENT IN AYURVEDA

Psoriasis is a disorder of skin called "Ek-Kushtha and by some authors as Sidhma Kushtha" in Ayurveda. It is described in ancient ayurvedic text of Ayurveda called Charak Samhita in the chapter of Kushtha Roga ( Skin Ailments ) and their Ayurvedic treatments.

According to Ayurveda, Psoriasis is Vata-Kapha disorder of Skin with involvement of Pitta and other 4 tissues called Rasa, Lymph, Blood and Skin. There are 18 types of chornic skin ailments described in Ayurveda and out of those 17 are said to be curable including Psoriasis. The combination of Skin diseases however can be thousands, but mainly 18 types are serious skin disorders as per Ayurvedic texts.

Charak Samhita is an ancient Ayurvedic text which is authored by Agnivesh ( A bright student of ancient sage - Atreya). Agnivesh is also known as Charaka by many. His time is around 1500 BC. So it's about 3500 BC old literature of Ayurveda explaining various complicated disorders including Psoriasis, herbs for psoriasis and various herbal compounds to get rid of Psoriasis permanently.

The main focus of the treatment is to normalize Vata- Pitta dosha, clean the blood and lymph and get rid of scales as fast as possible."

The treatment gives results within first month of usage and long term use of herbal remedies described in Ayurveda can completely heal the skin. Usually it can take up to 9 months to 1 year but the results are visible within few weeks of treatment.

Out of 18 types of skin ailments described in Charaka Samhita ( 1500 BC text on Ayurveda ) 7 are called Major and 11 are called minor skin ailments.

  9. Visphota Kushtha ( Folliculitis )

  11. Vicharchika Kushtha ( Weeping eczema )

In my opinion, Sidhma Kushtha should be considered as Psoriasis as in symptoms of Sidhma Kushtha mentioned in Charaka, it is said that the skin becomes whitish, silvery with fishy scales. Sometimes the scales are like copper and they are thin scales. If you rub them, some dust like powder drifts out of them. The scales are like flowers of a gourd family plant. This type of "Kushtha ( Skin Ailment) " usually occurs on upper part of the body.

Prognosis of Psoriasis in Ayurveda- In Ayurveda, Psoriasis is mainly considered as Vata-Kapha imbalance disease affecting many other cells / tissues of the body ( mainly skin). It is possible to get very good results and cure it using blood purifying herbs and herbs which work at cellular level to improve skin health and texture. Fortunately, there are many herbs which are working really well in Psoriasis.

DIET IN PSORIASIS

Ayurveda recommends very good diet to be followed in Psoriasis. Following the diet alone can give good results in the patients.

AVOID

Avoid citrus fruits, Long travel, Avoid spicy, fried food, Avoid soft drinks, All sour fruits like lemons, oranges, kiwi, pineapples should be avoided under all circumstances. Avoid Alcohol, Non-vegetarian diet. Butter, Milk and milk products should be avoided immediately. Avoid packaged food and avoid high protein diet. Salt intake should also be restricted.

ADVICE

Bitter vegetables like Long Gourd, Pumpkin, round gourd, Bitter gourd, apples, salads, fruits and vegetables ( green leafy).

AYURVEDIC TREATMENT FOR PSORIASIS

THE BEST COMBINATION WHICH REALLY WORKS IN PSORIASIS IS AS MENTIONED BELOW

Prescription which I usually give to Psoriasis patients in my clinic is-

#1. MIXTURE OF FOLLOWING

  1. Praval panchamrit- 5 gm
  2. Kalmegh Navayas Loh - 5 gm
  3. Swarn Makshik Bhasma - 10 gm
  4. Mukta Pishti - 2 gm
  5. Kamdudha Ras - 5 gm
  6. Yashad Bhasma - 5 gm
  7. Akik Pishti - 5 gm
  8. Jahar Mohra Pishti - 5 gm
  9. Giloy Satv - 10 gm 

Make 60 packs of equal dosages of all these mineral preparations, consume 1 dosage twice daily, with honey or with milk ( 10-20 ml).

#2. BLOOD PURIFYING HERBS FOR PSORIASIS

  1. Cap. Radiant Skin Hair Nail - 1 twice daily
  2. Cap. Neem - 1 twice daily
  3. Derma support tab (2 bottles) - 2 morning, 2 evening
  4. T-sora cap (240 caps) - 2 morning, 2 evening
  5. Immunoj (240 caps) - 2 morning, 2 evening
  6. Vasaka cap (2 bottles) - 1 morning, 1 evening
  7. Jeevantyadhi ghrit (2 bottles) - 1/2 spoon with milk
  8. Kushtkutar tab (240 tabs) - 2 morning, 2 evening
  9. Livopill (240 tabs) - 2 morning, 2 evening
  10. Kaishore guggul (2 bottles) - 2 morning, 2 evening

APPLY

  1. Vetpalai oil (4 bottles)
  2. Somraji ghrit (3 bottles)
  3. Epiderm oil (6 bottles)
  4. Wash with Aloe vera gel wash (2 bottles)

ALL this for 3 to 6 months.

Please take all the pills with coconut water. Results will be evident within 1st month of usage.
